Under the Vendor Services Development Initiative, CCNY portioned out funding to provide some area Care Coordination agencies with laptop computers, for easy portability, and real-time capturing of all their assigned family’s data, such as the Child & Family Team meeting minutes, Assessments, Plans of Care, and Crisis Plans, as well as for connecting those same families to the broader vendor network via the vendor search software.
CCNY will first use these laptops for targeted trainings for Vendor Agency supervisors and Care Coordinators, then assign them to Care Coordination agencies, for use with their wraparound families.
This added resource is expected to increase the effectiveness of each Care Coordinator, provide each family with real-time access to service providers, as well as real-time modifications to existing Plans of Care, Crisis Plans, and Planned Actions.Care Coordination agencies will be expected to sign a contract for their laptops, and the agency will be responsible for any repairs not covered under warranty, as per their contracted agreement.
